The UCSD Free Clinic UCSD Project (UCSD Free Clinic) is a non-profit free clinic that runs four community locations and is headquartered at San Diego University Medical School, La Jolla, California. The UCSD Student Free Clinic Project is one of about 24 student-managed clinical programs in the country; students plan, manage, and perform clinical operations under the supervision of a licensed physician. The UCSD Student-Free Clinic Project strives to provide high-quality and comprehensive care for uninsured and under-served patients throughout San Diego, who can not afford access to care; the majority of patients work poor. The four clinics are located at the First Lutheran Church at Downtown San Diego, Pacific Beach Church Unity Church at Pacific Beach, Baker Elementary School located north of National City, and Golden Avenue Elementary School in Lemon Grove.

Histori

Students at the UCSD School of Medicine want to start a free clinic for many years however, the model initially proposed does not get support from the UCSD administration. One day, a UCSD pre-medical student who had been on duty at the Luggage Clinic in Berkeley, California approached Dr. Ellen Beck, M.D, Clinical Professor in the Department of Family and Preventive Medicine at UCSD. Together, they meet with interested students and form a planning committee. The main learning for teams is that exclusive buildings are not necessary for clinic homes; the project requires an established community partner who can provide them with support and facilities.

The team identifies an established program that already serves the homeless at the Methodist Church of Pacific Beach United. Mary Mahy, a previously homeless woman, has created the Harvest for the Hungry program and accepted the idea of ​​a free medical clinic. With their support and assistance, the UCSD Student Clinic Free Project gets access to several rooms in the church that they can use during the evenings. Thus, the first UCSD Free Clinic-Free Clinic site opened at Pacific Beach Church Unity Church in January 1997. On that first night, 10 patients came for help and over the next few months the clinic became very busy.

A pastor of the First Lutheran Church in downtown San Diego, who is passionate about social justice and serving the homeless, hears about the free clinic that has started in Pacific Beach. She approached Dr. Beck and asked if the second site could start at his church. This coincided with the increasing interest of students for the project and with a sufficient number of medical student volunteers, the second site opened in October 1997.

Medical students are then attracted to reaching underserved minority populations. Initially, Dr. Beck approached an inner-city church. Although the congregation is mainly the middle class, the pastor of the church refers. Beck to his wife, Dr. Louilyn Hargett, who is a leader at Baker Elementary School. Dr. Hargett, together with the principal, Dean Krisi, welcomed the clinic making and became a partner with the UCSD Student Clinic Project. During 1998, the San Diego School District District approved a request to use the school building for a medical clinic. It is the school board's hope that the clinic will help to affirm the idea that schools are in the community center. On October 13, 1998, the third site of the UCSD Free Campus Student Project was opened at Baker Elementary.

During the first 10 years of operation, the clinic provided free medical care to 7,500 people. At any given time, the clinic has about 2,500 active patients. Clinical funds are from UCSD, grants, foundations, and donors.

Services and partners

The UCSD Student-Free Clinic Project offers many services in addition to basic medical care. This includes the care of specialists, dentistry, acupuncture, basic pharmacy, occupational and physical therapy, legal aid, and access to social workers.

Specialist Clinic

In an effort to provide comprehensive medical care, the UCSD Student-Free Clinic Project offers a specialized clinic where active patients have the opportunity to receive treatment from a specialist. A special clinic is an occurrence once a month that an active patient should be referred before the scheduled special clinic date. Patients from the UCSD free clinic may be treated by doctors representing: cardiology, dermatology, endocrinology, gastroenterology, gynecology, haematology/oncology, nephrology, neurology, ophthalmology, orthopedic surgery, podiatry, pulmonology, and rheumatology.

Dental Care

Dental care is the largest unmet health care requirement in the country; 108 million people do not have access to dental care. Restorative dental care of the patient, not only emergency dental care, is made possible by the work of UCSD Pre-Dental Society. UCSD students organize dentists to volunteer and treat patients on appointments except in Pacific Coast locations. Approximately 5-10 patients are seen during every clinic session. Due to the limited resources and availability of professional volunteer dentists, potential dental patients are usually placed on waiting lists until there is room for them to receive complete dental care except in the case of a dental emergency.

Acupuncture

Acupuncture is provided by Pacific College of Oriental Medicine. The acupuncture experiments are encouraged for pain and stress management as part of the clinic's integrative focus.

Drugs Provided

UCSD Students Skaggs School of Pharmacy volunteered at the UCSD Free Clinic Project run by Students under the supervision of a licensed pharmacist. The clinic's pharmacy distributes many types of free medicines to patients. When a patient needs a drug that is not sold pharmacies, drugs can be ordered through pharmaceutical drug donation programs. Pharmacies work closely with volunteers who manage to obtain drugs through the drug company's drug assistance program. While this allows free clinics to obtain medication for patients free of charge, there are many documents to be completed for each patient enrolled in the program.

Legal Advice

California Western School of Law (Cal Western) organizes Community Law Projects at the Downtown clinic. The Mission of the Community Law Project is to offer legal advice, information, and pro-bono referral services to low-income residents of the San Diego community. The law students from Cal Western work directly in a multidisciplinary context with other clinics at the UCSD Student Free Clinic Project to provide legal services to clients. The Community Law Project handles all legal areas related to poor clients, with more emphasis on Family Law, Consumer, Administration and Immigration. It does not provide legal representation.

Social worker consultation

Students from the San Diego State University school, an apprentice social worker with the UCSD Clinic Free Project Project run by a Student under the guidance of a social worker. They provide patients with resources and information about public programs, housing, public services, and employment opportunities.

Clinic

The Free Clinic Project aims to serve patients who are not eligible to receive health care through other programs.

The new patient clinic was held Monday night in the city center and Wednesday night in Pacific Beach. A medical student or medical student team conducts comprehensive interviews including information about the current condition of the patient, current history of the disease, social history and family. Physical examination of the patient is also performed. The medical student (s) then hand over the patient case to the attending physician and together they talk to the patient. Medical students and patients present collaborate to determine the best course of action. If needed, medicines are ordered from pharmacies and then distributed to patients.

Dampak nasional

The UCSD Student-Free Clinic Project created a national faculty development program in 1999 called "Overcoming Unserviced Health Needs" with grants. More than 107 lecturers from 30 countries have visited UCSD for three weeks to learn the mechanism of making free student-run clinical programs in their community. The UCSD Student-Free Clinic Project now offers an Advanced Skills Program, where previous participants return for a week and continue to learn from each other and their programs.

Through "Overcoming Unqualified Health Needs," and individual consultations, the UCSD Student Free Clinic Project Project has been replicated across the country in at least 7 new locations including Jackson, Mississippi; Lexington, Kentucky; Kansas City, Kansas; and Houston, Texas.

In addition, participants have used information from the program to create 29 new population curricula in community medicine, 18 new medical student curricula, successful grant proposals that address training in underserved communities, and new programs addressing the needs of migrant workers, homeless streets, and other under-served groups.
