The heel pad syndrome is a pain that occurs in the middle of the heel. This is usually due to the fatty pad atrophy that forms the heel. Risk factors include obesity. Other conditions with similar symptoms include plantar fasciitis. Treatments include rest, pain medication, and heel cups. It becomes more common with age.

Signs and symptoms

  • Pain in the heel, usually in the center of the heel. This is in stark contrast to the plantar fascial pain or heel spasms that are on the front of the heel, not in the middle.
  • Pain is usually a deep and blunt pain that feels like a bruise.
  • Pressing with your thumb into the center of the heel must recreate the pain.
  • The condition is often associated with a blow to the heel - landing hard when barefoot on a hard surface, jumping with shoes with hard heels, stepping on a rock while running.
  • Pain is aggravated by walking barefoot on hard surfaces such as ceramic tiles, concrete, hardwood floors, etc.

Diagnosis

The main differential diagnosis of heel syndrome is plantar fasciitis. On the heel pad syndrome all parts of the heel are soft while in plantar fasciitis usually only the heel part closer to the toes of the aching feet.

Treatment

Manual and exercise therapy has better efficacy over the long term than electrophysical agents and sports for function, but not for pain. Therapy and manual exercises are preferably focused on stretching the plantar fascia.

However, orthopedic legs do not seem to help with heel pain.
